Providing Everything You Need
Prior to the appointment you will be sent a form to complete with some basic details. This will allow some insight into current situation and how it is being managed and, in conjunction with the interview, allows us to make appropriate recommendations for your specific requirements.
Step1: Home visit Initial assessment.Approximately 40 minutes. We will discuss your everyday life and identify where help is required including potential solutions.
Step2: You will receive an email summary of the assessm and a personalised list of recommendations.
Step3: Follow up emails or telephone calls based on your specific requirements.
*If you prefer remote assessment rather than remote assessment, this can be arranged.
The following can be offered where clients require an ongoing support service
- Advice and guidance regarding applying for benefits, Concessionary travel (Blue Badge, Taxi card) and where required telephone support to manage the proce
- Full OT report
- Letter to GP, hospital, Local authority etc
- Attending hospital appointments
-Support discharge procedure from hospital where required
- We can also work closely with your care company in order to advise specific needs for your care plan.
